Protocole de prise en charge conjointe généraliste / spécialiste de l’infection à Helicobacter pylori à l’aide d’une fiche de recommandations : ressenti du médecin généraliste

Abstract : BACKGROUND: H pylori infections are managed by gastro-enterologists (GE) and general practitionners (GP) together. To improve this collaboration, GEFH developed a recommendation leaflet on the helicobacter.fr. website. OBJECTIVE The aim of this study is to report the feeling of GPs about this procedure. MATERIALS AND METHODS: A questionnaire was mailed to 875 GP of Savoies between July and November 2014. The reply rate was18% (n=111) and 90% of respondents had already managed Hp infection. 57 GPs did not know about the leaflet (pop 1) while 54 did (pop 2). RESULTS: The leaflet was considered clear (87% of respondents), easy to understand (95%), and gave the feeling they were working from evidence-based medicine (89%). GPs kept a paper version or a computer file. 98% of pop1 and 76% of pop 2 GPs thought they would refer to the document, especially for treatment protocols. Patients who went back to their GPs seemed to be more involved and more compliant (69%). 92% of GP’s believed that patients were not unhappy about seeing the GP again. GPs did not have the feeling that GEs were giving them additional work or that GE was delegating responsibility especially in pop 2 (87% vs 70% et 96% vs 81%, for pop 2 and pop 1 respectively, p<0.05), or that GEs consider GPs incompetent (95%). This collaboration GE/GP was easy to apply (95%), and 90% of GPs wished to continue working this way as they felt it was useful for the patient (88%) and that they were at the center of the management strategy (95%). CONCLUSION: The GEFH leaflet is a simple and very good tool for GE/GP collaboration and improves the management of Hp infection.
