Intérêt de l’étude de l’hémostase délocalisée par thrombo-élastographie (TEG®5000) dans les hémorragies du post-partum : étude rétrospective

Abstract : Background: Postpartum haemorrhage (PPH) may be complicated by a coagulopathy reflecting its severity. Our main goal is to study the correlation between TEG® parameters and standard coagulation assays. We also aim at analysing the performance of TEG® to detect a coagulopathy and to predict a severe PPH. Methods: In a single-center retrospective study, we included patients with PPH that were treated with sulprostone. During the first 24h, besides the standard coagulation assays (PT, APTT, fibrinogen according to the Clauss method (FibC), platelets), we performed a Kaolin and Functional Fibrinogen TEG® test. Results : We included 62 patients and performed 90 TEG® tests. There was a moderate to high correlation between the FibC values and FLEV/MRTGG-F/MA-K/MRTGG-F values and between the platelets count and FLEV/MA-K/MRTGG-K values (r=0.5-0.8, p<0.0001). After a multiple linear regression, MRTGG-F only depended on the FibC values and MRTGG-K depended on FibC and platelets count (p<0,0001). MRTGG-F<1.5 dcs could detect a hypofibrinogenaemia ≤ 2 g/l with high sensibility (85%) and specificity (90%) (AUC 0.94, p<0.0001). MRTGG-K<5.2 dcs could detect a hypofibrinogenaemia ≤ 2 g/l and/or a thrombocytopenia ≤ 80,000/mm3 with high sensibility (92%) and specificity (98%) (AUC 0.98, p<0.0001). The mean delay to obtain MRTGG-F and MRTGG-K was 3.1 min and 8.3 min respectively. MRTGG-F<3 dcs could predict a blood loss ≥ 3 l and a transfusion ≥ 4 CGR with high sensibility (79% and 74% respectively) and specificity (69% and 68% respectively). Conclusion: A significant correlation exists between the FibC values and TEG® parameters FLEV and MRTGG-F on the one hand, and between the platelets count and the TEG® parameter MRTGG-K on the other hand.
