Transmission verticale du virus de la dengue en période périnatale et au cours de l’allaitement : étude observationnelle réalisée au cours de l’épidémie de dengue de 2012-2013 en Nouvelle-Calédonie

Abstract : Objectives. Dengue is the most frequent human arbovirus. This study aimed to investigate vertical transmission of dengue virus during perinatal period and breastfeeding. Method. A prospective and descriptive study was carried out during the most important dengue epidemic in New Caledonia, in 2012-2013. Dengue virus was investigated and quantified by RT-PCR in pregnant or breastfeeding women (blood, milk), and their infants (blood, gastric liquid, cord blood, placenta). Clinical and biological evolution of these infected women and their new born was analysed. Results. Among 10 813 dengue cases registered, 10 women had positive dengue viremia during delivery. One premature birth, 3 haemorrhagic complications, and 1 maternal death were reported. Nine of the ten new born (90%) were contaminated. All were symptomatic, such as désaturation (4 cases), moaning (3 cases), feeding difficulties (3 cases), hypotonia (2 cases), skin eruption (1 case), blotches (1 case), one severe thrombopenia but not haemorrhagic, and one encephalopathy. Breastmilk samples were collected from 6 foregoing mothers, and 6 other breastfeeding mothers. Dengue virus was detected in 9/12 milk samples (75%). Conclusion. Vertical transmission of dengue risk is very high during delivery (90%), with major risk of obstetrical and neonatal severe consequences. Modes of vertical viral transmission are still difficult to understand precisely, but the risk of viral transmission through breastfeeding milk seems possible and cannot be ruled out. Newborn babies should be kept under longer surveillance to prevent dengue complications. Complementary studies are necessary to elaborate preventives recommendations.
