L'effet nocebo : étude préliminaire descriptive chez des patients hospitalisés en néphrologie

Abstract : The nocebo effect (NE) refers to the induction or the worsening of symptoms induced by sham or active therapies, caused by the patient’s negative expectations. It may have clinical consequences, lead to non-adherence and increase the costs of medical care. This study focuses on adverse effects (AE) reported by patients, their relationship with drugs and their information needs. The main objective is to determine the frequency of NE among the AE reported by patients. Secondary objectives are to determine a profile of patients at risk for NE with a questionnaire including questions about the relation of patients to their drugs and their personality, and to compare the information needs of patients and with those of health professionals. Thirty-five patients and 19 health professionals were included in the survey between May and August 2014, in the nephrology department of the Grenoble University Hospital. Patients have on average two AE, of which 12.7% are considered by pharmacovigilance experts as NE. Patients with NE are more anxious than those who do not have such effects. The NE sensitivity score defined through our questionnaire is significantly higher in patients with NE. The majority of patients feel that health professionals do not sufficiently explain the AE of their medication and they would like to know all the possible AE when a new drug is introduced. The information needs expressed by patients are coherent with those felt by health professionals. Identifying patients at risk of NE could prevent its occurrence and our questionnaire appears to offer a valid tool that could help to attain this objective.
