Quatre semaines de programme quotidien d’entraînement par intervalles avec récupération active pour améliorer la marche chez l’artériopathe claudicant : essai clinique randomisé

Abstract : Background: Supervised exercise training is part of the first line therapies for patients with intermittent claudication (IC), however the contain of the gait training remains to be determined. Treadmill training with active recovery has been proven efficient in cardiac rehabilitation. This monocentric trial was aimed at testing treadmill training with active recovery as a novel rehabilitation approach for IC. Methods and Results - Forty patients were randomized to either interval training with active recovery (ITAR) or interval training with complete rest (ITCR) program, 3 hours a day, for 4 weeks. The primary outcome measure was maximum walking distance (MWD) measured with a graded treadmill test before and after program. Adherence was high in both groups (>95%), and tolerance excellent (no adverse event). In the two groups, walking abilities were as such improved as with sixmonth programs, 3 days a week. The intensity and the energy expenditure of training were lower in the ITAR than in ITCR. ITAR program was similar to ITCR program in improving the maximum walking distance measured with a graded treadmill test and a Strandness test, and in changing the peak oxygen uptake. ITAR program appeared effective in increasing the perception in walking speed, but not ITCR. Conclusions: The interval training with active recovery might be of interest for PAD patients limited by IC at high cardiovascular risk, due to the lower intensity of the training, and for patients available for a short period of time (one month).
