Parametric Response Map as a new MRI approach for the characterization of post-traumatic cerebral edema

Abstract : Purpose: i) To characterize the nature of cerebral edema in a rat model of diffuse traumatic brain injury (TBI) using parametric response map (PRM) approach compared to classical averaged apparent diffusion coefficient (ADC) analysis; ii) to see whether early acquisition of PRM could predict the short-term evolution toward vasogenic or cellular edema after trauma. Materials and Methods: Eighteen rats subjected to diffuse TBI (TBI group) were compared to 7 sham-operated rats (control group). Diffusion-weighted acquisitions were acquired before (H.ref) and immediately after the insult (H0), and at 60 min (H1) and 120 min (H2) later. Mean ADC values and PRM analysis were performed in two regions of interest (ROI): cortex (cortical ROI) and whole brain (Brain ROI). The diagnostic performance of PRM acquired at H0 to predict the nature of brain edema at H2 was assessed using the area under the receiver-operating characteristic curve (AUC-ROC). Results: Compared to the sham-operated group, TBI group had posttraumatic edema in the cortex ROI using ADC analysis (p=0.04). No difference was found between the two groups where considered the brain ROI (p=0.32 versus sham-operated). PRM approach could detect the presence of posttraumatic edema even in the brain ROI (p<0.01 versus sham-operated). In addition, early PRM analysis could predict the evolution toward vasogenic or cellular edema at H2 (specificity 100%, sensitivity 89%). Conclusion: These findings indicate that PRM approach might be more accurate than ADC analysis to identify post-traumatic brain edema. A better knowledge of the nature of brain edema after TBI may help physician to personalize the patient management.
