État des lieux de l’activité de SOS Médecins Pau lors de la permanence des soins en médecine ambulatoire : étude rétrospective descriptive des visites en 2013

Abstract : SOS Médecins is a team of 6 general practitioners (GP) with paramedical training based in Pau, France, who respond to calls made to their own hot line (H24) and calls made to the local emergency service number 15 administered by the SAMU (C15). This study considers who the users of H24 are, and how they differ from those of the C15. The aim of this retrospective study which cover the period from January 1st to December 31, 2013 is to describe the physicians' home visits (PHV) and highlight differences between H24 and C15 users in periods when other GPs are closed (PDSA). Of the 4277 PHVs conducted in 2013, 14% were calls to C15 and 86% to H24. C15 users were mostly male with a mean age of 46. Their health and psychiatric care demands being mostly dealt with by health care service providers and administrative institutions in the Pau area. Patients who directly called H24 were younger (36 on average), mostly women and children living in 30% of cases in caravans or in social housing. Call were mostly for gastrointestinal illness and infections. The percentage of calls from outside the normal geographical operating area was 11% (C15: 2%, p <0.001). The majority of calls were moderate emergencies (C15 : 62 % H24 : 74 %, p <0.001) with a higher proportion of the highest levels of urgency in the C15 group (C15 : 15 % H24 : 5 %, p <0.001) which also led to a higher proportion of hospitalization (C15 : 6 % H24 2 %, p <0.001). In 75% of cases the diagnosis was consistent with the original reason for calling for both call centers. The study shows H24 users have different needs to those who call 15. We propose a series of measures to make more effective use of local resources, and relieve GPs in Pau and neighboring areas of the burden of PDSA work while providing a long term solution to unscheduled care demands.
