Le décubitus latéral : perspectives pour l'examen gynécologique du point de vue des patientes

Abstract : Introduction - More than 50% of women who should get pap-smear screening for cervical cancer are not or under-screened. The position in which pelvic examination is performed appears to be one of the explanations for under-screening. Alternatives do exist to the traditional lithotomy position, such as the left lateral position. We aimed to study patients' impressions about the left lateral position. Methods - We conducted a descriptive study with 114 patients, recruited by eight healthcare providers (general practitionners, gynaecologists, midwives) who are experts in practising the lateral decubitus position. Results and discussion - Patients' impressions of the left lateral position were positive: 59% found it comfortable, 79% little or not painful, 85% felt that anxiety generated by pelvic exam in this position was low, and 96% that it was respectful of their modesty. The impossibility to see the caregiver during the examination was mentionned by one out of three patients, and 45% of those referred to it as a positive point. Conclusion We showed that the left lateral decubitus position is an interesting alternative to the traditional lithotomy position, especially for patients whose coping strategy is avoidance (those who prefer not to see the examiner). Giving patients the opportunity to chose the position in which they prefer to be examined might decrease the anxiety associated with pelvic examination, and therefore improve the adherence to pap-smear screening.
