Étude descriptive de la prise en charge de la goutte en pratique de médecine générale dans le bassin annécien

Abstract : Background: Gout is the most common inflammatory joint disease in men. Both prevalence and incidence are increasing these last forty years, due to a change in people’s supply and population’s aging. Its management is difficult, particularly with old persons and/or having one or several comorbidities. The study’s aim was to evaluate General Practitioners instruction habits when gout occured in Annecy’s region, and compare them to the last internationals guidelines published (American College of Rheumatology in 2012 and European League Against Rheumatism in 2006), as well as to obtain an epidemiologic evaluation concerning gout. Methods: A prospective, descriptive study was achieved, by sending an original questionnaire to the physicians, questioning them about their instruction habits in case of gout, and gout epidemiology. Results: 84.1% of physicians prescribed a monotherapy in case of acute gout, among them 73% prescribed colchicine and 23% non-steroidals anti-inflammatories. Concerning urate-lowering treatments (ULT) in case of chronic gout, 40.9% of physicians proposed an alternative to allopurinol, historical ULT. 43.2% of asked physicians estimate that between 50 and 75% of their gouty patients had one or several comorbidities. Conclusion: Despite recent publication of guidelines about gout, asked physicians seemed to have kept their past habits concerning drug treatments. However, diagnosis and non-drug treatment were well managed by them. Due to a difficult management (frequently bound comorbidities) and increasing prevalence, this potentially invalidating pathology should be highlighted by health authorities, as well as promoting both primary and secondary care.
