Prévalence de l'obésité chez les femmes enceintes et les implications sur le déroulement de la grossesse dans la population sud-basse-terrienne : étude descriptive, rétrospective

Abstract : Objective: To evaluate the deleterious effects of maternal obesity on obstetrical complications and neonatal outcomes in the population of Caribbean women.
Patients and methods: Retrospective, cohort study including all patients delivered in the maternity of Basse Terre Hospital Center between May 2012 and April 2013. Women were divided into five categories by the Body Mass Index at the beginning of gestation: ≥18,5 to <25 kg/m2, ≥25 to <30, ≥30 to <35, ≥35 to <40, and ≥40. Maternal epidemiologic data, obstetrical complications, labor complications, and neonatal outcomes were studied.
Results: Among 774 patients, 48,7% was of normal weight, 23,2% was overweighed and 20,2% - obese. Class II obese patients were five times more likely to have personal arterial hypertension history and those with class III obesity – seven times more likely to have diabetes mellitus type 2 history. The prevalence of gestational diabetes mellitus, macrosomia and cesar section were significantly more frequent in obese patients. The total of 42,8% patients were conforming to Institute of Medecine 2009 weight gain guidelines. 74,5% of obese patients was multiparous.
Discussion and conclusion: Our study confirms the influence of obesity on increase of metabolic and obstetrical complications in obese pregnant women. Taking into account the higher proportion of obese women entering the pregnancy and greater effect of multiparity on weight retention in Guadeloupe, the ensemble of health care professionals and public authorities should strengthen the actions against obesity.
