Exercice de la médecine générale avec ou sans électrocardiogramme

Abstract : Introduction : Many general practitioners provide primary care without electrocardiogram (until 50% according to studies). A general practitioner faces a clinical situation which needs an electrocardiogram on average once a week. Method : Our study, led between June and July 2013, compared how 38 general practitioners cured their patients according to their equipment or non-equipment in electrocardiograph. The study population was represented by the tutors of the department of general medicine of the Bordeaux faculty (France). Results : 62 patterns of consultation have been collected. Indications brought to 80% of emergency patterns like thoracic pain, heart rhythm disorder, faintness/blackout, dyspnea. 20% of indications were on cardio-vascular risk factors management and certificate for non contra-indication of sport practice. Data interpretation led to 3 tendencies: absence of electrocardiogram seems to increase the transfert rate of the patient and the emergency nature of the transfert at the end of the consultation, absence of electrocardiogram seems to increase the rate of prescription for complementary exams, absence of electrocardiogram seems to reduce the rate of therapeutic prescriptions (introduction and turnover). Discussion : Recommandations offer the possibility to practice without electrocardiogram in primary cares but encourage its realization. This study does not let define the absence of electrocardiogram as the only limiting factor for patients which needs one in primary care. Conclusion : Absence of electrocardiogram seems to limit the medical autonomy in primary care. Major complementary studies are needed to bring to light statistically significants datas.
