État des lieux et pistes pour optimiser la vaccination contre le virus de l'hépatite B au centre pénitentiaire de Gradignan (Gironde)

Abstract : The HBV vaccination is indicated for prisoners who present risk factors of contamination. The immune status of prisoners at the beginning of incarceration and from six to twelve months later has been studied. The objective was to optimize the immunization coverage with the introduction of a monitoring process. Patients and methods : Two audits have been made with an interval of exactly one year. Data were collected retrospectively from medical records of prisoners being jailed from six to twelve months. Results : The study included 231 patients. Groups were similar, except for this which included patients who accepted to be screened at the beginning of incarceration. The prevalence of excessive drinking was 25.9% and the prevalence of drug addiction history was 18.1%. The rate of HCV RNA positive prisoners was 5.8% and the rate of HIV positive prisoners was 1.6%. Among the patients, 29.9% were born in a area of medium or high endemicity for HBV, which is significantly associated with the fact of being exposed to it. There were 57.1% prisoners immunized against HBV : 42.9% because of a previous vaccination, 14.3% because they had been exposed. There was a correlation between being under 30 and being vaccinated. Susceptibility to HBV infection decreased from 49.0% to 31.0% in 2013 and from 36.0% to 23.6% in 2014. The time between the first day of incarceration and the day of screening had been reduced from 22 to 18 days (p=0.013). The median time between incarceration and the three vaccine injections did not change. Conclusion : Susceptibility to HBV infection decreased significantly from about 50% to less than 25%. It shows an important population protection after the introduction of the monitoring process.
