Évaluation de la transmission des données médicales en médecine générale : Cas du changement de médecin traitant

Abstract : Introduction : medical records are all documents concerning the patient's health held by his doctor. It guarantees the quality but also the safety of the care of the patient. Although mandatory, the transmission of medical data, when changing doctor treating seems insufficient. Objective : the main objective of our study was to clarify the medical records supplied by the patient during the first contact with his new doctor. The secondary objective was to assess the impact of chronic or potentially serious nature of the disease or diseases , the transmission of all or part of the medical record. Methods : it was a survey of practice, during two months , with 400 general practitioners randomised, established in Gironde. At the first consultation with a new patient , the practitioner must complete a questionnaire , to be returned by mail. This questionnaire allowed to inform the doctor treating the profile (sex , exercise ), the patient profile (age, gender , reason for change general practitioner, concept of chronic illness or long-term illness ) , the contributed papers by the patient ( medical, prescription , a specialist courier , the result of diagnostic testing and vaccination ), the notion of contact between the last and the new general practitioner and content of medical records when it was transmitted. Results : 98 responses were usable. The transmitted medical data are insufficient when a patient change of doctor treating . In chronic disease, this transmission is better but still insufficient. On average, less than one third of patients send their medical records. Practitioners who retire more transmit their medical information to patients. The alert data ( intolerance allergy) , screening and vaccinations are often absent from medical records, when transmitted to the new practitioner. Conclusion : a new patient who change of general practitioner, even in cases of chronic disease, provide few medical data to their new general practitioner. The potential consequences of this lack of information should induce a reflection in order to improve the transmission of such data.
