Étude en médecine générale : poursuite de la prescription de statines chez les patients de plus de 80 ans institutionnalisés dans les Landes en 2014

Abstract : The current statins prescription on subjects older than 80 years-old in France is based on the recommendations established by the AFSSAPS in 2005. No prospective multicenter study has been realised on this specific population: prescription is mostly based on younger adults data. Our study's aim was to describe, in 2014, statins prescriptions on subjects older than 80 years-old living in Mont-de- Marsan retirement homes, and to judge if the treatment was still necessary. Secondary objectives were to assess the prevalence of secondary dyslipidemia, clinical and biological monitoring, and to achieve the therapeutic goals set by the recommendations. On the 754 residents of the nine nursing homes, the study included 100 patients consuming statins and the mean age was 87 years. They had an average of two cardiovascular risk factors including age. Patients with OMS-score equal to 4 were 28%, and equal to 3 were 58%. The latest and most powerful molecules were most prescribed, atorvastatin and pravastatin, and regrouped 43% and 31% of prescriptions. To keep the prescription was questionable in 34 of 43 patients in primary prevention, and in 35 of 57 patients on secondary prevention. The most common causes of secondary dyslipidemia were iatrogenic and kidney. In spite of 46% of patients who had no lipid monitoring in the year, about 80% of patients were in goal set by the recommendations for LDL-cholesterol. For 13% of patients the dosage or the molecule had to be reconsidered because of very low LDL-cholesterol value, less than 0,7 g/L. According to the current studies, non-drug measures of dyslipidemias management (adapted physical activity, mediterranean diet) has to be promoted, and could limit the dyslipidemias impact in old aged people.
