La maîtrise de stage en médecine générale ambulatoire : points de vue de médecins n'ayant jamais reçu de stagiaire, analyse du manque d'attractivité de la fonction

Abstract : Qualitative study through semi-structured interviews with family doctors from Ile-de-France. The increase in number of general medicine residents and the reform of the second cycle and its ambulatory training course which became compulsory, complicate the task of universities’ general medicine departments which struggle to find areas of training for these students. They have to deal with reluctance from practitioners given the decrease in number of private practices. This work’s objective is to analyze the reasons for the lack of appeal of mentorship. It is a qualitative study based on partially guided interviews with nineteen general practitioners in Ile-de-France who have never worked with residents in their practice. The analysis reveals that they are mainly afraid of the time-consuming aspect of mentorship. A fear of judgment from a third party interfering with the "doctor-patient relationship" also appears. They recognize, however, that mentorship could improve their practice. For the oldest, trained "on the job", internship has no real interest. The youngest, who made it, recognizes its benefits but do not wish to become mentors in turn. Our European and Quebecker neighbors are not facing with the same reluctance. In their countries, they are close ties between university faculty and physician mentors who claim to be scholars (academics) themselves. The French doctors’ ambivalence toward internship might result from confusion between universities and hospitals and from a feeling of exclusion from the academic world, due to a lack of early exposure to general medicine during the studies.
