Traitement chirurgical des diverticules de zenker : résultats à court et long terme à propos de 70 cas

Abstract : Objectives: Treatment of Zenker’s diverticula may be surgical or endoscopic. The objective of this study was to assess results of a series of patients treated by surgical management. Methods: We retrospectively reviewed 70 patients (43 men and 27 women, mean age 73,5 years, range: 16-92 years) consecutively operated for esophageal diverticulum between 2001 and 2012. The surgical technique was a diverticulectomy associated with esophageal myotomy and wide calibration. The immediate and distance results were analyzed and compared with published series of patients treated endoscopically. Results: Clinical manifestations were dysphagia (98.6%), regurgitation (45.7%), false roads (4.3%), weight loss (21.4%), and pneumonia (7.1%). There was no postoperative death and morbidity was 17.1% (12 patients). The average recovery of oral feeding and post-operative daysn were respectively 3 and 8.2 days. The Long term evaluation (mean follow 2 years, range, 9 months-11 years) show 62 (88.6%) patients satisfied with the operation and 8 (11.4%) patients with partially improvement. An analysis of published series of endoscopic findings showed a shorter duration of hospitalization but less good results in long term. Conclusion: The Functional long-term results could be better after surgical myotomy and diverticulectomy rather than endoscopic treatment.
