Apport de la scintigraphie de perfusion cérébrale dans l'exploration de la démence sémantique

Abstract : Objective
Semantic dementia (SD) is a clinical syndrom with proeminent language disturbance, belonging to primary progressive aphasia (PPA).
There is numerous diseases underlying this clinical syndrom including Alzheimer's disease (AD) and fronto-temporal lobar degeneration.
The aim of our study was to investigate brain perfusion differences among DS subgroups according to their cerebro-spinal fluid AD profiles.
Methods
20 patients with a SD clinical diagnosis and who underwent lumbar puncture with measures of biomarkers (BM) and brain perfusion SPECT where retrospectively included.
DS patients where classified in two subgroups according to their tau/Abeta42 ratio : SD+ (with an AD CSF profile) and SD-(no having an AD CSF profile). The clinical diagnosis was made while blind to the biological classification and to the brain perfusion SPECT results.
The brain perfusion profiles of the SD subgroups where compared between each other and with those of 24 healthy subjects and 24 patients with a typical amnestic MA.
Results
SD patients had left-side predominant hypoperfusion in the temporal poles and among them those with an AD CSF profile had a more severe hypoperfusion in the left inferior parietal cortex. Compared to the typical amnestic MA, the SD+ had hypoperfusion in the left temporal pole.
Conclusion
Distinct hypoperfusion patterns in SD BM positive and SD BM negative patients where observed. Parietal hypoperfusion in SD patients must be considered atypical for a FTLD and must discuss the underlying pathology.
