Évaluation de la connaissance du statut vaccinal coqueluche : enquête réalisée auprès de 2 maternités à Paris et 373 patientes en France en 2014

Abstract : Introduction: Bordetella pertussis is increasingly prevalent in France (fatality rate of 1.7% before 3 months old). Most common contaminators are parents and siblings. Cocooning strategies still aren’t widely applied; this may be because both doctors and patients are unfamiliar with the patient’s vaccination status. Method: A questionnaire aimed at pregnant women in two Parisian maternities determined what patients knew of their bordetella pertussis vaccination status and how they objectified the information. Factors predicting a lack of knowledge around vaccination statuses were also studied. Results: 225 questionnaires from the Bluets maternity and 148 from the Pitié Salpêtrière were retrieved from 01/06/14 to 31/08/14: 36.95% of patients didn’t know their vaccination status for bordetella pertussis; in only 23.06% of cases, patients brought their vaccination records. Social security (p=0.0063), education levels (p=0.0028) and geographical origins (p=0.0002) were determining factors in the patients’ lack of information on vaccination statuses. The age of patients, however, was not a determining factor (p=0.09). Conclusion: Many women still don’t know the status of their bordotella pertussis vaccine. This may partly be why cocooning strategies are difficult to set‐up in France, especially for patients who have recently arrived in France, those with additional medical treatments and those with low levels of education. To improve follow‐up for this vaccine, healthcare professionals could be trained to retrieve vaccination status information, provide more information to patients, use a computerized medical care system, and perhaps vaccinate systematically when in doubt.
