Impact d’un guide thérapeutique sur la prise en charge à domicile de la douleur des patients en soins palliatifs : utilisation du cas vignette

Abstract : A leaflet was created by the RESPALIF (Fédération des Réseaux de Soins Palliatifs d’Ile de France) to gather recommendations on pain in palliative care. This non randomized study between two groups of general physicians working in Ile de France who received or not the leaflet Douleur En Ville: évaluation-Aide à la prescription pour les patients en situation palliative, realized from April to August 2013, with a clinical vignette, in order to seek if the leaflet’s use impact pain control in palliative care. The leaflet’s possession doesn’t intervene on negative PMI (non progression in the pain ladder in case of step 2 painkiller’s failure): “leaflet group” 23,2% versus 12,2% in the “control group”. The global score of French and European recommendations’ accord isn’t modified but the score is high (81,6%). The shift between oral morphine and transdermic fentanyl is known by only 40% of physicians. Belonging or not to a palliative care network, gender, number of annually followed palliative care patients, the use or not of the leaflet doesn’t impact the negative PMI or the global score of recommendations’ accord. The elder physicians have significantly less negative PMI than the youngest but the global score of recommendations’ accord is similar.
