La septoplastie endoscopique : revue de la littérature, technique chirurgicale, courbe d'apprentissage

Abstract : Introduction: The aims of our study were : 1) to compare endoscopic septoplasty versus traditional septoplasty, 2) to describe the endoscopic surgical procedure and our way to perform it and 3) to study the learning curve. Methods: We did three studies on endoscopic septoplasty. The first study was a literature review, comparing endoscopic septoplasty versus traditional septoplasty in terms of surgical time and functional results. The second study was a description of our surgical procedure, which consist of 12 steps. The third study described the learning curve of the endoscopic septoplasty using surgical time of the first 100 patients of a senior surgeon. Results: In the literature review, we listed 5 prospective and randomised studies comparing endocopic septoplasty versus traditional septoplasty. The operative time was shorter with the endoscopic procedure, p<0,001. There was no difference between the 2 groups in terms of functional results. Our surgical technique has 12 steps: diagnostic endoscopy, sub-perichondral infiltration, anterior mucosal incision, dissection of mucosal flap on the left side, cartilage incision, dissection of the mucosal flap on the right side, removal of the anterior part of the cartilaginous septum, dissection of posterior part of the osseous septum, removal of maxillary crest, final endoscopic control, suturing and placement of silicone splints. The learning curve showed that a surgeon need 60 cases to reach an operative time of 21 minutes. Conclusion: Endoscopic septoplasty shorten operative time compared to conventionnal technique, with the same functionnal results. This technique has 12 key points. After 60 procedures, the endoscopic septoplasty can be quickly perform by an experienced endoscopic surgeon.
