Facteurs émotionnels, cognitifs et comportementaux dans la fibromyalgie

Abstract : INTRODUCTION: According to a biopsychosocial model of fibromyalgia (FM), psychological and behavioral factors should be considered, in order to better understand and treat these patients. METHOD: A literature review is performed by screening the MedLine database over the period between 2000 and 2015. RESULTS: Anger, sadness and anxiety aggravate the painful perceptions. Pain is considered as a chronic stressor. Lazarus and Folkman’s transactional model of stress can be applied to pain, and thus establishes a link between pain and emotional suffering. Catastrophizing is regarded as a maladaptive coping strategy. Some avoidance behaviors and, conversely, excessive movement persistence appear to aggravate FM. The attachment style and alexithymia are key aspects of the personality that must be considered in the clinical assessment and management of fibromyalgia patients. These psychological and behavioral factors account for integrated therapeutic programs, that are made of pharmacological treatments, patient education, appropriate physical activity and psychotherapies. Psychotherapies aim to improve stress management and to increase self-efficacy through active patient involvement in the care. Some technics (Cognitive and Behavioral Therapy, Mindfulness Therapy, Acceptance and Commitment Therapy) focus the dysfunctional coping strategies (catastrophizing, kinesiophobia, hyperactivity) to lessen their impact and to develop more functional coping strategies, such as distraction, relaxation and acceptance. DISCUSSION: These results confirm the importance of psychological factors in the development and maintenance of symptoms of FM. They must be considered in the treatment of FM.
