Chirurgie endoscopique de l'adénome hypophysaire : qualité de vie rhinologique

Abstract : Introduction: Endoscopic transsphenoidal pituitary adenoma surgery is currently considered a minimally invasive approach. Few clinical studies have examined the rhinologic outcomes following this surgery. This study aims to evaluate the rhinologic quality of life (QOL) before and after 3 months postoperatively. Methods: It was a single-center prospective study. All patients undergoing endoscopic pituitary adenoma resection were consecutively enrolled from November 2014 to March 2015. Before surgery and 3 months postoperatively, assessment of QOL with the 22-Item Sino-Nasal Outcome Test (SNOT 22), nasal symptoms with Visual Analogue Scale (VAS), and odor threshold test (Sniffin Sticks Test) were performed. Postoperative nasal endoscopy findings were confronted with the SNOT 22. A repeated measure ANOVA was developed to determine factors predicting a worse QOL. Results: Thirty-six patients were eligible for study. There was a significant worsening in the rhinology-specific mean SNOT 22 scores at 3 months postoperatively (p < 0.05) while postoperative mean SNOT 22 scores showed no significant difference from preoperative scores. After surgery, no changes in symptom scores (VAS) were observed except for the loss of smell (p <0.05). The endoscopic presence of edema, mucopurulence and crusting had a tendency to worse QOL. No factors correlated with worse QOL. A loss of smell was significantly reported at 3 months postoperatively (p<0.05). However, no difference was found by the odor threshold test. Conclusion: The rhinologic quality of life is deteriorated 3 months after endoscopic transsphenoidal pituitary adenoma surgery. Patients should be informed before surgery.
