Le devenir des embryons surnuméraires après transfert frais

Abstract : Objectives: In general, good quality embryos are chosen in order to be transferred and cryopreserved at the cleavage stage (day 2 / day 3), poor-quality embryos (PQE) are usually destroyed. The aim of our study was to evaluate the issue of prolonged culture of supernumerary embryos whatever their quality at day 2, and to determine the implantation and evolutive pregnancy potential of the blastocysts derived from PQE. Material and Methods: Retrospective study conducted from November 1st, 2012 to July 1st, 2014. The primary endpoint was the total blastulation rate (total number of blastocysts obtained at Day 5 and Day 6 whatever their quality) and the useful (total number of good quality blastocysts frozen at Day 5 and Day 6). Secondary assessment criteria were the clinical pregnancy, birth and miscarriage rates. Results: 3108 embryos were maintained in extended culture (1319 obtained from IVF, 1789 from ICSI). The total blastulation rate was equivalent in IVF and ICSI (56.6% vs 53.2%). In contrast, the rate of useful blastulation (blastocysts frozen at day 5 + day 6) was significantly higher in IVF (28.7% versus 25%). The majority of frozen blastocysts derived from embryos having an optimal kinetic at cleavage stage in the two groups (73.6% vs 76.3%). The clinical pregnancy rate was 39% in IVF vs 43.2% in ICSI. The birth rate was significantly higher in ICSI (20.5% versus 50%) but with an equivalent number of ongoing pregnancies (14.4% vs 20.8%). Conclusion: Our study confirms reported data in literature about the morphological evaluation of embryo quality. The blastocysts are mainly obtained from good quality embryos at day 2. Nevertheless, blastulations and births could be obtained from EMQ.
