Déterminants des retards de prise en charge du dépistage de la tuberculose chez l'enfant

Abstract : Screening of pediatric TB is a public health issue. The main objective of our study in consultation of tuberculosis in Robert Debré Hospital in Paris was to test whether the treatment of children who had contact with tuberculosis case index, was in accordance with current recommendations and determine the appropriate management of delay factors.59 children were included in our study of the period from 01.01.2013 to 01.04.2014.The demographic characteristics of our population were close to the national characteristics. Most of the delay was located in the pre-hospital phase and involved in our study population age 5 years and more specifically those under 2 years. The delay factors to support identified in our study were: the small age and more specifically age less than 2 years, the assumption by the CLAT, the lack of significant medical history, the presence of at least one risk of tuberculosis disease factor and the fact provide symptomatic tuberculosis. A delayed care was not associated in our study with a higher risk of developing tuberculosis disease. The main limitations of our results were the small size of our population and the study of a journey to our own hospital, difficult to generalize to the rest of the population. A possible solution to im-prove the delay of the management of these children would be to more involved city stakeholders as the attending physician.
