Implementation and impact of telethrombolysis in acute ischemic stroke at the mutualist clinic in Lesparre-Medoc

Résumé

Stroke is a pathology that affects 130,000 people each year in France, which can result in severe sequel, or death. The mutualist clinic of Lesparre-Medoc will be able to offer, in 2016, an increased chance of recovery to victims thanks to the advent of thrombolytic therapy for ischemic stroke. The objective of our study is to characterize the population in Medoc suspected of having a stroke and to assess the number of patients likely to benefit from thrombolysis via telemedicine at the mutualist clinic of Lesparre-Medoc . The study was conducted retrospectively on the years 2013 and 2014. We have observed, on 2 years, 157 patients suspects to submit a stroke, 90 of which were indeed an ischemic stroke. The median age was 81 years, the population was mainly psychopathology and polymerize. The main brake on the telethrombolysis was a delay symptom-admission excessively long. In total, 15 patients were eligible for an telethrombolysis. If we include the patients suspected of stroke of the territory served by the EMS of the clinic of Lesparre-Medoc but addressed to the universitary hospital in Bordeaux by the regulation of the SAMU 33 of the fact of the current absence of thrombolysis, we found ourselves 8 additional patients who could have been thrombolysed in this center of care. The clinic of Lesparre-Medoc may hope thrombolyse a patient by months of the operation of the thrombolysis, and even more in the light of the aging of the population. Nevertheless, efforts with the patients are to be done in the area of primary and secondary prevention of cardiovascular risk, of the recognition of the symptoms of stroke as well as of the importance of consultation in emergency in case of occurrence of these symptoms. In this, thrombectomy, available at the universitary hospital in Bordeaux, offers an additional opportunity to treatment of stroke for patients that don't meet the criteria of thrombolysis.
L’AVC est une pathologie qui touche 130 000 personnes chaque année en France, pouvant entraîner de lourdes séquelles, voire le décès. La clinique mutualiste de Lesparre-Médoc va pouvoir offrir, dès 2016, une chance accrue de récupération aux victimes grâce à l’avènement de la thrombolyse des AVC ischémiques. L’objectif de notre étude est de caractériser la population médocaine présentant une suspicion d’AVC et d’évaluer le nombre de patients susceptibles de bénéficier de la thrombolyse via la télémédecine à la clinique mutualiste de Lesparre-Médoc. L’étude a été menée rétrospectivement sur les années 2013 et 2014. Nous avons observé, sur 2 ans, 157 patients suspects de présenter un AVC, dont 90 présentaient effectivement un AVC ischémique. L’âge médian était de 81 ans, la population était principalement polypathologique et polymédiquée. Le principal frein à la téléthrombolyse était un délai symptôme-admission excessivement long. Au total, 15 patients étaient éligibles à une téléthrombolyse. Si l’on inclue les patients suspects d’AVC du territoire desservi par le SMUR de la clinique de Lesparre-Médoc mais adressés au CHU de Bordeaux par la régulation du SAMU 33 du fait de l’absence actuelle de téléthrombolyse, nous retrouvions 8 patients supplémentaires qui auraient pu être thrombolysés dans ce centre de soins. La clinique de Lesparre-Médoc pourra espérer thrombolyser un patient par mois dès le fonctionnement de la téléthrombolyse, voire plus au vu du vieillissement de la population. Néanmoins, des efforts auprès des patients sont à faire en matière de prévention primaire et secondaire du risque cardio-vasculaire, de la reconnaissance des symptômes d’AVC ainsi que de l’importance de consulter en urgence en cas d’apparition de ces symptômes. En cela, la thrombectomie, disponible au CHU de Bordeaux, offre une opportunité supplémentaire de traitement de l’AVC pour les patients ne pouvant répondre aux critères de la téléthrombolyse.
