Facteurs prédictifs de la réponse à un programme de réentraînement à dominante aérobie chez des patients fibromyalgiques

Abstract : Introduction: Fibromyalgia (FM) is a chronic syndrome characterized by widespread pain and tiredness. As a result, patients often deal with a lack of physical activity and a poor fitness level which take part to their symptoms and their global disability. The aim of this retrospective study was to identify the potential predictive factors of a good self-perceived clinical improvement, after completing an aerobic training program. Methods: We conducted a retrospective study on 185 FM patients who underwent a 12-week aerobic multicomponent training program (3 sessions a week). Patients' self-perception of clinical evolution at the end of the entire program was scored with the Patients Global Impression of Change (PGIC): Responders (PGIC ≤ 2) and Non Responders (PGIC ≥ 3). Baseline patients' characteristics were analyzed to determine predictive factors of a good self-perceived clinical improvement. Results: Older patients (p=0.016), a treatment with Acetyl-Lcarnitine (p=0.011), non-steroid anti-inflammatory consumption (p=0.013), and current employment (p=0.017) were four significant explicative factors to characterize patients who felt relieved after completing the training program). A trend (p=0.06) was found between a fewer non opioids painkillers utilization and a good response to training. No physical fitness or cardiovascular parameters were included in the model. Conclusion: ln FM, pain management and current working unemployment seem to play a more important role than fitness at baseline to explain clinical improvement induced by a training program. These results should be taken into account in order to adapt training program for patients with poor probability of self-perceived improvement.
