Défibrillateur automatique implantable en cardiologie pédiatrique et congénitale : revue de la littérature, expérience bordelaise et perspectives d'avenir

Abstract : Pediatric and congenital heart disease (CHD) population is concerned by sudden cardiac death (SCD). Gold standard treatment in prevention of SCD by ventricular arrhythmia in adults is implantation of a cardioverter-defibrillator (ICD). Efficacity of ICD in children and CHD patients is proved, even if there is less literature about this subject than in adults. This is a specific population, as morphologically than anatomically or physiologically. Besides, due to their young age and progress in CHD domain, they are “well” exposed to ICD-related complications. Since the first implantation in adult (1980) and young adult with CHD (1989), there has been a lot of evolutions in ICD, in technology and implantation approach. We aim to make the point about ICD-implantation in children, adolescent and young adult, with and without CHD, to describe differents implantation modalities, their respective advantages and drawbacks, present local experience in this field, las technologies, psycho-social impact of ICD-implantation and future perspectives.
