Critères diagnostiques et scores d’activité en médecine interne : des travaux préliminaires du PHRC INFections et IMmunité à leur utilisation en pratique clinique

Abstract : Background and objective: A regional hospital clinical research program to determine the risk factors for infections in patients suffering from autoimmune diseases (MAI) treated with immunosuppressants was selected for funding in 2013. Preliminary work on this study led us to conduct a detailed analysis of diagnostic criteria and activity scores. These activity scores are, when they exist, debated, complex and regularly challenged. As part of the project, a practical survey was performed to analyze the use of the activities scores in daily practice and their determinants. Methods: The team performed developed a questionnaire about activity scores concerned concerning systemic lupus erythematosus, Sjogren’s syndrome, autoimmune myositis and necrotizing vasculitis of small vessels. We interviewed practitioners on the frequency of use utilization of these tools and, the cause of non-use utilization, if anywhen relevant. Results: 92 internists have completed the form. 70% of them supported the utilization of the activities scores in consultations. Actual utilization is less than 25% of patient visits in the majority of cases. The reasons for the low use of these scores are mainly ignorance of their existence (42%) and their length or complexity (28%). Actual activity criteria used in consultations are in turn very variable and imprecise. Conclusion: The available activity scores are not used in routine practice. Clinicians are however very favorable to their utilization. There are indeed a number of needs for them in particular: limit the subjectivity of the practitioners, foster communication, allow retrospective studies. These needs are not filled with the current available scores.
