Évaluation de l’état de santé, des risques professionnels et de l’exposition aux métaux d’une population de prothésistes dentaires

Abstract : AIM: To describe the health condition with a focus on breathing of a population of dental technicians, and to study the correlations with occupational exposures. METHODS: The targeted population consisted of dental technicians affiliated to RSI Isère and neighboring departments, together with technicians employed in their laboratory. They answered a detailed questionnaire about their occupational exposures, as well as the collective and individual protections currently used. A metric quantifying the number of prostheses produced was calculated: prostheses-years. Biological exposure indices (BEI) concentrations (urinary chromium, cobalt, nickel, cadmium and beryllium) were measured at the beginning and the end of week. The subjects had a medical consultation with PFT and chest radiography. RESULTS: 31 managers had a consultation and 42 technicians were included in biological monitoring. A former exposure to asbestos which was serious enough to advise a post exposure monitoring was demonstrated for 16 managers and 1 employee. Alloys with beryllium are still used by a quarter of tested technicians. MEF and Tiffeneau ratio are negatively related to the number of prostheses-years after taking into account age and smoking: R = -0.56 (p = 0.023) and R = -0.63 (p = 0.004) respectively. BEI concentrations are similar to expected values observed in general population. Although measured levels do not show a significant difference between the beginning and the end of the week, the total number of implants performed during the week significantly affect the urinary concentrations of the end of week (p <0.05). DISCUSSION: This work describes the working conditions and the health status of a population of technicians. Atmospheric measurements of crystalline silica would improve the assessment of the risk of pneumoconiosis.
