Traitement symptomatique d’une céphalée aiguë de l’adulte dans les services d’urgences en France : enquête de pratique, recherche bibliographique et proposition de protocole anticipé

Abstract : Headaches are a common reason for consultation, whether in primary care or in emergency department. In Europe, they can represent up to 16% of the consultation in emergency department and about 4.5% in primary care. The aim is to quickly identify secondary headaches due to a serious condition. However, patients are often so painful and uncomfortable, that it is not possible to proceed to a specific interrogation and a good quality clinical examination, leading to a possible delay to the characterization of headache in primary, secondary benign or potentially serious secondary headache. We conducted a prospective survey in France, interesting ED of 42 French hospitals. A questionnaire was developed to assess patients treatment for primary headache disorders in the ED. 75% of services respondents, lack of early protocol for the management of acute headache. Paracetamol and NSAIDs are the treatment administered as first-line in 50% and 33.3% of cases respectively. Nevertheless, the literature has demonstrated the effectiveness of Metoclopramide and oxygen therapy in the management of headaches. The addition of Metoclopramide has demonstrated its effectiveness as analgesic headache, treatment of associated signs and improve the efficacy of others treatments. The oxygen seems to be an interesting option for the symptomatic management of all headache regardless of its cause. We proposed an early protocol for the management of all acute headache presenting to the emergency without prejudging its etiology, in order to improve the diagnostic approach.
