Place et apport de l’ablathermie dans le traitement des métastases pulmonaires du cancer colo rectal : étude rétrospective sur 209 patients pendant 12 ans

Abstract : Background: To evaluate the location of lung thermal ablation in the treatment of metastatic colorectal cancer and her results in terms of overall survival (OS), disease-free survival (PFS) and chemotherapy-free survival (CFS). Patients and methods: Between 2002 and 2013, the prospective database of our center including all consecutive patients treated with thermal ablation for lung metastasis of colorectal cancer was retrospectively reviewed. Results: Two hundred nine patients including 109 men (52%), 69 [26,2 ; 86,4] years old with primary tumor to the colon in 50,2% and rectum in 49,8%, received 323 thermal ablation for 630 lung metastases. Lung metastasis were new in 87,9%, missing metastases in 6,7% and local progression at the site of ablation in 5,4%. Radiofrequency ablation was the most used (96,9%) with micro-waves (2,2%) and cryothérapy (0,9%). The median diameter of metastases was 10 mm [2 - 46]. 63,2% and 24,4 % of patients respectively had one or 2 thermal ablations (maximum 5 : 1,4%). Median follow-up was 50 months. Median OS was 67,6 months. Median PFS was 7,6 months. OS and PFS rates were 95% and 33,1% at 1 year ; 54,7% and 11,2% at 5 years. Median CFS survival was 12,2 months. Extra pulmonary metastasis were independently associated with a reduced UCS. Conclusion: Thermal ablation for colorectal lung metastasis is able to extend the break without chemotherapy during 12,2 months and to increase survival improving the patient life quality.
