Impact de la réhabilitation précoce post-opératoire sur le ressenti maternel et la satisfaction vis-à-vis du lien mère-enfant après une césarienne

Abstract : Introduction : the objective of our study was to investigate the maternal impact of an enhanced recovery program after cesarean delivery, on the feelings and satisfaction towards creation of the mother-child bond, in comparison to a classical postoperative care. Methods : a comparative, prospective and multicenter study was conducted in three maternity units: the Robert Debré maternity unit applies standard traditional postoperative care (Paris, France), while the maternity units of Bicêtre hospital (Val-de-Marne, France) and Hospital Intercommunal Créteil (Val-de-Marne, France) apply an enhanced recovery program. Were included patients after elective or emergency caesarean delivery who had given birth to full-term healthy singleton newborns (n=86). Data were collected from 8th October 2014 to 31st January 2015. Patients were asked about their feelings toward the relationship with their infant using a questionnaire, to be completed one and three days after cesarean delivery. Results : patients received post-operative care in agreement with what was expected in the group in which they were included. Patients in the «enhanced recovery program» group had more positive feelings toward the relationship with their newborn on the first and third postpartum day in comparison with patients of the «classical care» group. Likewise, they had a greater maternal satisfaction level one day after cesarean delivery and were more comfortable in caring for their newborn, especially when cradling and breastfeeding the child. Conclusion : our study suggests that application of an enhanced recovery program after cesarean delivery is associated with improved satisfaction and more positive maternal feelings toward the relationship with the newborn.
