Les polymorphismes génétiques du TRKB : influence sur l’efficacité des antidépresseurs chez des malades déprimés

Abstract : Introduction: To better predict the answer to the antidepressants is a major stake of public health, in terms of quality of the care and economic terms. The Receiver Tyrosin-Kinase B (TRKB) is the receiver of Brain Derived Neurotrophic Factor (BDNF) and plays a significant role in the regulation of the neurogenèse. It is implied in certain psychiatric pathologies. The action of the antidepressants depends partly on the neurogenèse and certain genetic polymorphisms of the BDNF have an impact on their effectiveness. The effect of genetic polymorphisms of the TRKB on the effectiveness of the antidepressants is less known. Our main aim is to evaluate the impact of 8 genetic polymorphisms of the TRKB (rs1778933, rs1187352, rs2289658, rs2289657, rs2289656, rs3824519, rs56142442 and rs1439050) on the answer and the remission after treatment by antidepressant among depressed patients. Methods: Within the framework of a prospective naturalist troop (METADAP) on a duration of 6 months, 624 adult patients of a French population presenting a major depressive episode (EDM) of unipolar type and requiring the introduction of an antidepressant treatment in conditions naturalists of regulation, profit from a genotyping and a clinical evaluation to inclusion and after 1.3 and 6 months from antidepressant treatment. Results: The subjects have a 45.6 years median age, 69.4% are women, it acts of a first EDM for 25.6% of them. We do not observe a difference according to 8 genetic polymorphisms of the TRKB concerning the sociodemographic characteristics and the clinical characteristics of the depressive disorder to inclusion. Differences in effectiveness of the antidepressants are observed according to 3 genetic polymorphisms (rs1439050, rs1187352, rs3824519). However none of these differences persists after corrections of Bonferroni nor after application of a procedure of replication. Conclusion: We do not find impact of 8 genetic polymorphisms of the TRKB on the effectiveness of the antidepressants. Future explorations should go on the Copy number variation (CNV) and the interactions between polymorphisms (haplotypes) and class of antidepressant treatment.
