Patients précaires diabétiques de type 2 : intérêt d’une consultation dédiée. Étude à la Permanence d’accès aux soins de santé de l’Hôpital Saint-André de Bordeaux

Abstract : Deprived subjects are reported to have an increased risk of diabetes and related complications, more serious than in general population. Objectives. To describe the characteristics, the health status, and the management of diabetic patients followed in the PASS (A healthcare center for patients with precariousness). Then, to estimate the efficiency of a dedicated consultation on glycemic control of patients with type 2 diabetes (DT2). Methods. Over a period of one year, we led a work in two parts. At first, a descriptive study of all the received patients with diabetes. Then, we selected patients with DT2 to estimate an intervention, including a simplified therapeutic plan and an individualized education. Results. Between April 2013 and April 2014, 23 patients with diabetes were met, with a majority of type 2 (n=20). The mean age was 53,6 years and the diabetes evolved for more than 5 years for 12 patients. Only 13 patients had benefited of at least one of the elements of the French guidelines. Patients with DT2 accumulated several cardiovascular risk markers: 78,3 % were obese or overweight, 70 % had high blood pressure, and 35 % were smokers. Their diabetes was uncontrolled. The eligible patients to the protocol had an average HbA1c of 9,6 %. At the end of the follow-up, an average reduction of 2 points of HbA1c was observed. Conclusion. Despite the difficulties, specific diabetes care model leads to improvement in health status of type 2 diabetic patients with deprivation. The therapeutic education is essential to develop.
