La part et les motifs des prescriptions des médecins généralistes dans les consultations des orthoptistes

Abstract : Background : Orthoptics is a paramedical profession evolving and easy to access, there are no studies on the relationship between general practitioners (GPs) and orthoptists. Method : 3 surveys were conducted. The first, quantitative, retrospectively analyzing medicals prescriptions of 15 orthoptics practices of Gironde. The 2nd, qualitative, via a phone questionnaire of GP found in the first survey, analyze their sociodemographic data and their relationships with orthoptists. The 3rd assesses the proportion of Gironde GP having prescribed orthoptic prescription in the last six months via a question by email. Results : On 1744 prescriptions analyzed 21.85% come from a GP. The main motive of GPs prescriptions is related to a binocular disorder or heterophoria (BDH) (51.97%). The others main precriptions are related to postural disorders and neuro-visuals disorders. The proportion of GPs amblyopia prescriptions, for children, in screening without suggestives symptoms is lower than pediatricians (p<0.0001). 74.54% of BDH prescriptions come from an ophthalmologist, 21.36% from a GP. 165 GPs responded to the questionnaire. Contacts with orthoptists before (36%) during (22%) and at the end of the treatment (systematic report 44%) are low. GPs between 40 and 60 years old, trained in homeopathy, in low APL area (localized potential accessibility) and working in médicals groups practices are more represented relative to the data in the Gironde. 51.34% of Gironde GPs who responded to the email answered having prescribed to an orthoptiste in the last six months. Conclusion : MG prescribed more than imagined to the orthoptist, often limited to DBH or for a reason not yet validated as postural disorders. Direct relations between them are rare especially in MG <40 years. DBH management and amblyopia screening are potentials improvement axis.
