Étude Hélirun, épidémiologie de l'infection à Hélicobacter pylori et de la résistance à la clarithromycine à la Réunion en 2014 : étude chez 200 patients adultes ayant une indication d'endoscopie digestive supérieure au CHU de Saint Pierre

Abstract : Background: In France,in 2012, the emergence of antibiotic resistance of Helicobacter pylori has produced new treatment guidelines (appearance of sequential therapy and quadruple bismuth treatment of first intention). The objective of this study was to determine the prevalence of H. pylori infection and the prevalence of resistance to clarithromycin in patients who underwent upper gastrointestinal endocoscopy in southern University Hospital of Réunion. In Réunion island, the latest épidemiological studies of H. pylori were performed in 1999. Method: This descriptive cross sectional study, conducted at the University Hospital of Saint Pierre from January to June 2014, included 200 adult patients (18-83 years) with an indication of upper digestive endoscopy and who have never received treatments for H. pylori infection. A patient was considered infected with H. pylori if he had a positive diagnosis in Histology and / or PCR. The clarithromycin resistance was determined by PCR. Results: A prevalence of H. pylori of 44.5% was obtained. We found a statistically significant association between infection and female gender (p = 0.008), and age of patients (p = 0.019). The under 30 years old were the least infected patients (21%) and most were infected over 70 years (64%). The prevalence of resistance to clarithromycin was 12,5%. Conclusion: H. pylori infection seems to have decreased on Réunion Island since 1999 (61% of prevalence). The prevalence of clarithromycin resistance has progressed but remains lower than that found in France. According to French and international recommendations, the use of standard triple therapy could always be indicated in Réunion island in first-line treatment, where the prevalence of clarithromycin resistance is low (<15%).
