Étude rétrospective bicentrique évaluant la survenue de complications et leurs facteurs de risque durant la grossesse des femmes atteintes d'une cardiopathie congénitale

Abstract : Background In women with congenital heart disease, pregnancy is associated with cardiologic, obstetric, fœtal and offspring complications. Our aim is to evaluate the occurrence and predictors of adverse events. Methods Retrospective bicenter study from July, 2009 to July, 2014. Seventy-nine women with congénital heart disease with 93 pregnancies were included. Data were collected on the obstetric and cardiologic file before, during and after the pregnancy. Results Cardiac event occured in 20,4% of pregnancy, obstetric, foetal and offsprings events occured in 66.7%, 7.9%, 33.7% of pregnancy respectively. Many predictors of the adverse events were identified significantly (p<0.05) like NYHA functional class, prior cardiac events, cardiologic medication, WHO classification, the CARPREG score, and left ventricular systolic function under 60%. Conclusion Maternal and neonatal adverse events are frequent in pregnant women with congenital heart disease. Prior evaluation of risk by a muldisciplinary team with symptom and risk index is important to managed their pregnancy.
