Analyse des principaux motifs de plaintes ordinales déposées à l'encontre des médecins généralistes dans la région Rhône-Alpes ces sept dernières années

Abstract : The filing of a complaint in medicine is a hot topic, which sometimes has an impact on our practice. The main objective was to analyse the motives of the patients' complaints to general practitioners, in order to prevent their occurence in a more efficient way. This retrospective observational study included 110 patients ordinal complaints against general practitioners (GPs), in the Rhône-Alpes region from 2008 to 2014. They were classified by types of complaints, grouped into six categories. The primary outcome was the identification of predominant categories of complaints. Secondary outcomes were changes in number of complaints over 7 years, and complaints from other individuals than patients (among colleagues for example), the exposure of other specialists to complaints' filings, sanctions, and outcome in case of appeal procedure. The "administrative" complaints were the majority (41%), because they included the reason "certificate" (31% of total). Followed in descending order by "medical" complaints (28%), then "human" causes (26%) including the reason "attitude / behavior" (19% of total), and "organizational" causes (5%). Over seven years, there was no notable evolution in the number of complaints. In this study, the Medical Board's patient complaints against general practitionners are primarily administrative (certificates) or due to an inappropriate behavior. Communication with the patient is therefore a key part of prevention. Complaints certificates merit further investigation to fully understand their characteristics. This phenomenon of complaint filing does not appear to increase over the years.
