Directives anticipées et patients atteints de sclérose latérale amyotrophique, analyse de contenu. Étude mutlicentrique dans les centres de référence de Bordeaux et de Toulouse entre le 1er janvier et le 30 juin 2015

Abstract : Background: Although advance directives (AD) appeared in France with the Leonetti law 10 years ago, very few patients write them. Amyotrophic lateral sclerosis (ALS), a neurodegenerative incurable and fatal disease, appears to be a paradigm of palliative care and ALS patients seem suitable to writing AD. The aim of this study is to analyze the content of ALS patients’ AD. Methods: A descriptive observational retrospective multicentric study with quantitative and qualitative analysis was performed in ALS reference centers of Bordeaux and Toulouse from January 1st to June 30th 2015. Results: Among the 600 included patients, 35 had written AD. All AD concerned “somatic” topics, mainly “tracheotomy” and “resuscitation”. There was a wide disparity in AD’s contents, depending on the writer: patients evoked many more topics, notably “psychic”, “existential” and “ethic”, than doctors. Conclusion: Our results suggest it is very difficult to talk about and write AD, both for patients and for caregivers. More research is needed in order to assess practical AD utility and use, on the one hand in decision making concerning heavy medical acts, on the other hand in global pain management.
