Modalités d’utilisation de l'isocinétisme par les clubs français de football professionnel et évaluation isocinétique des muscles extenseurs et fléchisseurs du genou chez le footballeur de haut niveau du Football Club des Girondins de Bordeaux à l’aide d’un protocole type

Abstract : Background: Hamstring strain is the most common injury observed for football players. Since the 1970's, muscle imbalance has been pointed out as a risk factor. Isokinetic testing on football players (installation & preparation) to detect muscle imbalance has to be performed under specific conditions. But the significant number of assessment protocols and the actual cutoffs make the studies quite impossible to compare. We sent the French Professional Football Clubs a survey to ask them about their current practice. A "MODEL" assessment protocol stands out, but the actual ratios and cutoffs to define a "NORM" are very unsimilar. We found interesting to use this "model" protocol on high-level football players to define them an Isokinetic Profile. Objectives: Draw up the French Professional Football Clubs different Isokinetic applications. Apply this "model" protocol (standing out the survey) on high-level football players since the age of 17 to define their Isokinetic Profile. Methods: uring the month of June 2015, a survey has been sent. The data has been collected over the end of August 2015. To define this Isokinetic Profile, FCGB Football Players have been included at the beginning of the season, when they were actually able to play football without any restrictions and to pass Isokinetic testing. Players who had lower limb strain over the past6 months had been excluded. Results: Over the 62 clubs requested, 52 answered; Isokinetic is widely used (for 94% of them). A model protocol stands out (42% apply it): « A Concentric testing at 60 and 240°/s for Q and hamstring and an Eccentric test for hamstring at 30°/s ». In a second study, reference values for cutoffs defined as pathological on bilateral and unilateral metrics and cutoffs are: RB: >15%; RC60°: < 0.51; RC240°: <0.64; RM : <0.83. Conclusion: Isokinetic is widely used by French Professional Football Clubs. This is why we recommend, in the future, the application of this "model" protocol, using the cutoffs indicated above, to detect muscle imbalance.
