Réhabilitation occlusale d'un patient polytraumatisé : protocole de prise en charge pluridisciplinaire

Abstract : The global dental treatment must be seen in a larger context and the manducatory apparatus must be put in perspective with the whole body system. The patient who undertook this work had multiple facial and orthopaedic fractures. In order to deal with the grievance of a patient subject to multiple traumatic injuries, "a disturbed occlusal balance" and unstable ATMs, we look for mechanisms that will allow a comfortable occlusal balance for the patient, and identify its influence on the rest of the body, while taking into consideration the adaptation of each patient. Simultaneously to the standard occlusal techniques, we try to determine the impact of additional techniques such as posturology, osteopathy or the "stabilimetre platform". The aim of this protocol is to assess whether, other than a better oral condition, we observe an overall improvement. The patient's feeling is sought a year and a half after the first consultation. He reports an efficient and physiological chewing function and his cervical pain has disappeared. Even if we cannot not say with complete certainty that these improvements are to be attributed solely to our protocol, the technical specifications resulting from the patient's grievance have been almost fully achieve. We offer a pedagogical approach oriented towards multidisciplinary care. The summary diagram shows guidelines which must be adapted to each patient, when conventional techniques appear to have reached their limits.
