Dépistage du syndrome d’apnées-hypopnées obstructives du sommeil dans l’hypertension artérielle résistante : quel est le rôle du pharmacien d’officine ?

Abstract : The “Hospital, patients, health and territories” law has permitted the chemist, local health provider, to take place in the health care screening and process of the patient, especially in chronic pathologies.The obstructive sleep apnea-hypopnea syndrome (OSAHS) is a true matter of public health, because of its prevalence and consequences on living conditions. The OSAHS is responsible for metabolic cardiovascular problems such as resistant hypertension (RH). The OSAHS remains under-diagnosed whereas there are simple screening tests available like the Berlin Questionnaire and Epworth Sleepiness Scale. In these circumstances, the purpose of the DEPISAHT study was to evaluate and put in place a screening strategy of the OSAHS directly at the dispensary for patients suffering from HR. Among the 17 enrolled drugstores showed in the study, Berlin Questionnaire and Epworth Sleepiness Scale were submitted to 69 patients taking at least 3 antihypertensive drugs. Overall, 30% of the patients were tested positive thanks to the Berlin Questionnaire. This test being 80% sure, it would therefore be able to detect 1 person out of 4 in our population. In common practice, this screening test is possible back at the dispensary. In one hand, the Berlin Questionnaire seems to be a quick and effective tool for the chemist and easily understood by patients. On the other hand, chemists are highly motivated (69%) to change their organization to implement the OSAHS test as a pharmaceutical mission.
