Évaluation d'une intervention de santé publique : dispositif de prise en charge des consommateurs de drogues injectables au CEPIAD, Centre de Prise en Charge Intégrée des Addictions de Dakar

Abstract : Context: Safe usages of injection drugs are very low in Senegal, where the social context toward this population is repressive and the health care limited. The National Strategic Plan in Response to AIDS 2011-2015 has included “injecting drug users” as a vulnerable population and has promoted the creation of the Center for Integrated Management of Addictions in Dakar (CEPIAD) within the enclosure of the National University Hospital of Fann in Dakar during the month of December 2014. Objective: The objective of the study is to assess the services provided to injecting drug users in CEPIAD in Dakar in 2015. Methods: We carried out a cross-sectional descriptive study in CEPIAD, from March to August 2015. Evaluation grids were used to measure users’ satisfaction, and evaluate the harm reduction project and the services offered. Data analysis was based on the technique of strengths and weaknesses. Results: During the study period, 180 injecting drug users including 10 women accessed the CEPIAD (1/7 consumers in Dakar). These consumers were tested for their HIV and HCV serological status (75% and 66%), received a social interview (179) and participated in friendly activities (91). 74% of the respondents said that they were satisfied with the services of the center. During a year on the field, the outreach team approached more than one third of the 1324 consumers from Dakar. Among the 512 consumers approached, the injectors received an average of 187 syringes, a quantity close to the 200 syringes needed per year and per injector according to WHO recommendations. Conclusion: The combination of various approaches and services enabling harm risk reduction among drug use has made CEPIAD the first dedicated center in West Africa. Effective integration of medical care (tuberculosis, HIV, HCV) and the accessibility of women into activities need improvements.
