Participation à l'adéquation entre besoins médicaux et offre de soins : influence des motivations personnelles sur le choix professionnel selon les futurs médecins généralistes : étude qualitative auprès de 17 internes en médecine générale

Abstract : Background: General practice has been in constant evolution for 50 years and has become a speciality. Despite of this promotion, demography is declining and medium-term projections are pessimistic. These statistics don't justify that working as a GP has changed and that some graduates tend to choose specific exercises. It also affects the healthcare system. Personal motivations are involved, although they are not due to purely individualistic aspirations. We found it interesting to study the influence of personal motivations in the career choice according to general practice trainees and to put them in perspective with professional motivations. Method: Qualitative study by semi-structured interviews with general practice trainees TCEM 3 of the Aquitaine region, via an interview grid. Results: From a personal perspective, family appears to be the most influencial element for interns concerning their professional project. It has an impact on the quality of life but it implies two kinds of work commitments: geographical and planning. By the partner and his professional needs, and by children. All trainees want to reconcile their career and their private life very well. They don't give up their exercise, and they adapt it to reconcile professional and private life desires. Trainees who choose a liberal professional activity rely on flexibility and independence. Those who choose a paid employment think they have enough time to reconcile everything. Working as a locum is considered as a temporary situation allowing the building up of a project. Conclusion: The kind of activity chosen by trainees is not just influenced by individualistic aspirations but depends on the family background and geographical offer. In this context of demographic pressures, it implies an analysis and solutions going beyond the health framework and repartition policy.
