Quels sont les facteurs prédictifs de survie dans l’arrêt cardiaque réfractaire nécessitant l’assistance par circulation extracorporelle type ECLS ? Étude rétrospective

Abstract : The use of extra-corporeal life support (ECLS) in refractory cardiac arrest (RCA) has spread in the last twenty years. However, morbidity and mortality associated with this practice remains high. Previous studies identified several risk factors, leading to recommendations in 2009. Our study looked for factors linked to survival of patients who underwent ECLS during RCA or shortly after recuperation.This epidemiological study is descriptive, and uses a monocentric and retrospective approach based on the archives of Grenoble's CHU between 2006 and 2014. A total of 196 patients who underwent ECLS after RCA or shortly after RCA recuperation was included in the analyses. The 30 days survival was the main variable, and the 3 month neurological status as well as the rate of ECLS-induced complications were also taken into account. The overall survival rate was of 15.8 %. The mean age of the patients was 49.4 years old. Low-flow duration was significantly lower for survivors than non-survivors (42,7 vs 70 min). There was no difference between in-hospital and extra-hospital cardiac arrest. Neurological status of the survivors was evaluated on 28 patients (5 were lost to follow-up), 23 of which had none or minor sequelae (CPC score 1 or 2), and the remaining 3 suffering heavy neurological damage. we recommend the criteria to start such procedure to be more restrictive.
