Analyse de 83 cas de réactions psoriasiformes sous anti-TNFalpha : étude rétrospective et proposition d'un score de sévérité photographique

Abstract : Background : Psoriasiform eruption is a classical skin adverse effect of anti-tumor necrosis factor alpha (anti-TNFa traetment). Objectives : We sought to describe and classifiy psoriasiform eruptions under anti-TNFa and to search for a link between the type of eruption and the underlying disease. Methods : Retrospective monocentric study of patients seen in the department of Dermatology of Bordeaux between January 2nd 2012 and May 1st 2015 and presenting a psoriasiform eruption under anti-TNFa treatment for various inflammatory diseases. When aviable photographs of patients were graded for severity (0, 1, or 2) by three independent blinded dermatologists form 6 types of psoriasiform reaction (psoriasiform plaques, scalp/skin fold involvement, generalized /palmoplantar pustulosis and palmoplantar hyperkeratosis). Results : 83 patients were included of whom 55 had aviable photographs at the first visit. Patients with inflammatory bowel disease presented significantly more often scalp (p=0,009) and skin lesions (p=0,002) involvement. Conclusions : Patients with inflammatory bowel diseases treated with anti-TNFa agents have significantly higher risk of severe and frequent psoriasiform eruption, especially in scalp and skin fold areas.
