Création et évaluation d’un dépistage informatique automatisé du risque d’escarre chez les patients hospitalisés

Abstract : Introduction: Pressure ulcers constitute a major public healthcare concern. Their prevention is necessary and starts with the screening of high-risk individuals. Objective: The main objective of this work is to reduce the prevalence and incidence of pressure ulcers through the creation of a new screening tool for high-risk individuals. Methods: It is a new computer-based, automated assessment scale for detecting pressure ulcer risks, based on a sophisticated algorithm computing a score from the patient registration form without increasing the workload. It generates a two level risk alert signal, aimed at warning caregivers in the event of a risk being detected. A measurement of the evolution of the rates of incidence and prevalence of pressure ulcers was conducted six months before and six months after the new scale was set up. The scale was assessed and compared to other means of prevention used in the hospital. Results: In 4903 patients, prevalence is at 3.9% and incidence at 0.8%. They remained unchanged during the study. But this new scale has an important area under the ROC curve (AUROC=0.924), a strong sensitivity and specificity. It shows strong concordance with the other means of screening and improves both the sensitivity of the risk assessment through the Braden scale as well as the prescription of rentals of alternating pressure air mattresses. Conclusion: A new original, automated scale for screening patients at risk for developing pressure ulcers was created. It contributes above all to sensitize caregivers to the risk of pressure ulcers without increasing their workload in contrast to the usual risk scales. It appears to be superior or at least comparable to the reference scales. But our mono centric study of a limited duration, with a small-sized sample, was not able to demonstrate a reduction of pressure ulcers. Further research studies would be necessary to validate this scale.
