Le laryngectomisé partiel‎ : états des lieux de sa qualité de vie après chirurgie

Abstract : The study of quality of life (or QoL) has really improved over the last twenty years. It deals with various fields, medicine included. The will to offer a quality medical follow-up and an always better one, has favoured the multiplication of life quality measuring instruments. Unfortunately, all medical disciplines have not benefited from this evolution, otolaryngology in particular. In fact, few surveys deal with the future of patients, especially as regards partial laryngectomy patients (PL patients). Aims : estimate the quality of life of partial laryngectomy patients in a cancer context and try to establish a comparison between the quality of life of PL patients and total laryngectomy patients (TL patients). Method : we have assessed the global quality of life as well as the quality of life according to the type of surgery of a hundred patients in the otolaryngology services of the “CHU Pellegrin” and the “Clinique Saint Augustin”, Bordeaux with two surveys (FACT H&N and Recueil de données psychologiques). The survey was made at least six months after surgery. Results : however long after partial laryngectomy, the global quality of life was relatively satisfactory (fifty percent of the patients were satisfied in each globally estimated dimension and this for each type of surgery, apart from the social and emotional dimensions more particularly altered even long after the operation. As regards the phonatory function, the patients are still affected (71%) and mention social and familial problems of communication, which are the result of vocal affection. The facts that we have collected concerning the QoL were the age of the patient, the time of the operation and, at the time of the survey, the cancer context, the type of surgery and the social and familial situation. Conclusion : in spite of the functional improvement and a globally satisfactory everyday life, the quality of life is still hampered in its social and psychological dimensions, even quite a long time after surgery. So as to help people after cancer to a better life maybe we could exploit our results (without any sense of pretence) in a way which we still have to define, in order to help the professionals of health (with clinically objective data) and the patients of course (preparing them psychologically before surgery for a better acceptance of post surgery aftereffect).
