Évaluation des facteurs prédictifs d'allongement du temps de passage aux urgences du centre hospitalier d'Agen

Abstract : Background: Our objective was to determine the factors of emergency department (ED) length of stay (LOS) over 4 hours. Methods: This was an observational, monocenter study over 5 months. All patients were included. Univariate and multivariate analyses were performed on the probability to have ED LOS over 4 hours. Results: Of 13280 attendances, 3063 had ED LOS over 4 hours. Median was 144 minutes. Multivariate analysis found signifiant predictors for ED LOS (p <0.001) to be: age (<15 (OR: 0.52 (95% CI 0.42 to 0.64)); 15-49 years, OR: 1; 50-64 years, (OR 1.44 (95% CI 1.18 to 1.74)); 65-74 years (OR 1.63 (95% CI 1.27 to 2.06)); 75-84 years (OR 1.87 (95% CI 1.46 to 2.39)); ≥ 85 years (OR 2.50 (95% CI 1.98 to 3.16)); arrival mode (ambulance, OR 1.49 (95% CI 1.29 to 1.73)); acuity level (level4-5, OR 0,89 (95% CI 0.80 to 0.99)); x-ray examinations (OR 1.39 (95% CI 1.24 to 1.55)); CT/IRM/ultrasound (OR 5.93 (95% CI 4.93 to 7.13)); admission to a ward (OR 1.23 (95% CI 1.09 to 1.38)). When number of patients were more than 18, and ⩽2 doctors (OR 3.64 (95% CI 2.10 to 6.21)); ]2;3] doctors (OR 2.91 (95% CI 2.4. to 3.49)); ⩾3 doctors (OR 1.33 (95% CI 1.13 to 1.54)). Conclusions: ED LOS seem to be related to patients’ factors and management. Mutualizing SMUR and emergency teams could be a solution to overcrowding.
