Randomized study to compare two modalities of online teaching for inspection diagnosis of perianal lesions of Crohn among gastroenterology residents

Résumé

Perianal lesions of Crohn’s disease (PLOC) are a diagnostic, therapeutic and prognostic challenge. Every gastroenterologist should learn to recognize them. Our hypothesis was that using pictures would improve learning. The aim of this study was to compare the diagnostic ability of french gastroenterology residents of the two different e-learning on PLOC, and then to compare residents to experts. Residents received a survey with 12 pictures of PLOC and closed answers. After randomization in 2 groups (group A received an online teaching with typical pictures and PLOC’s definitions and group B definitions only), residents answered a second survey with 12 other pictures of PLOC. We requested 19 experts to choose typical pictures of each PLOC from a picture’s bank and answer survey 2. The investigators established the survey quotation bareme. Our primary endpoint was the comparison of the average score for survey 2 between the two groups of residents. The second endpoint was to compare residents to experts. Two hundred fourty eight residents among 465 answered survey 1, then 195 survey 2. The groups A and B got the same average score of 71,9/100, while the experts got 74,6/100 without statistical difference with the residents. This study didn’t show any difference between the two teaching methods. This might be due to the quotation bareme or the inspection limits for PLOC’s diagnosis.
Les lésions ano-périnéales de la maladie de Crohn (LAPMC) constituent un enjeu diagnostique et pronostique. L’objectif principal était de comparer une formation en ligne iconographiée à une formation identique sans iconographie, pour la reconnaissance des LAPMC à l’inspection par des étudiants en Diplôme d’Enseignement Spécialisé d’hépato-gastro-entérologie de toute la France ; puis de comparer les résultats des étudiants à ceux d’experts. Les étudiants étaient invités à répondre à un 1er test avec 12 photos de LAPMC. Après randomisation en groupes A et B, ils recevaient une formation e-learning comportant les définitions des LAPMC dans les 2 groupes et des photos typiques des LAPMC seulement dans le groupe A. Ils remplissaient ensuite un 2e test, qui constituait le critère de jugement principal. 19 proctologues experts en LAPMC élaboraient le e-learning et répondaient au 2e test. Les grilles de cotation des tests étaient conçues par les investigateurs. Deux cent quarante-huit étudiants sur 465 ont répondu au test 1, puis 195 au test 2. Les 2 groupes obtenaient une note moyenne de 71,9/100 points ; les experts une moyenne de 74,6 ± 4,3 (IC95% [72,1 ; 77,1]) sans différence statistiquement significative avec les deux groupes d’étudiants. Le support iconographique ne permettait pas aux étudiants de mieux reconnaitre les LAPMC à l’inspection. Cela pourrait être lié à des limites de notre barème de cotation ou aux limites de l’inspection du diagnostic des LAPMC.
